How to prepare a records transfer

Patients may be asked to authorize records from prior clinicians. Final request instructions, fees when applicable, and the secure destination will be posted when practice systems are active.

  • Identify the offices, hospitals, and specialists holding important information.
  • Request only records relevant to your current care when possible.
  • Use the secure portal, fax, or another approved method published after the practice systems are active.
  • Do not send medical records by WhatsApp, general email, or social-media messaging.

Information to understand before scheduling

Insurance and self-pay

Bring a current photo ID and insurance card when applicable. Insurance participation and benefits must be verified; coverage is not a guarantee of payment.

Cancellation and no-show policy

Please call as early as possible to cancel or reschedule. The final cancellation and no-show timeframe can be updated here after the practice policy is confirmed.

Late arrivals

Patients who arrive late may need to wait, receive a shortened visit when clinically appropriate, or reschedule so other appointments are not delayed.

Prescription refills

Request routine refills before medication runs out. Some medications require a visit, monitoring, or laboratory review before renewal. Refills are not handled through general email or WhatsApp.

Laboratory and imaging results

Results will be communicated through the secure patient portal, telephone, or another approved method. Timing depends on the test and clinical importance. General messaging is not appropriate for urgent concerns.

Medical-record requests

Patients may be asked to authorize records from prior clinicians. Final request instructions, fees when applicable, and the secure destination will be posted when practice systems are active.

Safe communication

General email, website messages, and WhatsApp are for basic scheduling only. Do not send symptoms, diagnoses, medication lists, insurance cards, photographs, or other private medical information through those methods.

Urgent and emergency care

This website and general contact methods are not monitored for emergencies. Call 911 or go to the nearest emergency department for severe or potentially life-threatening symptoms.
